Perimenopause is the transition, often starting in your mid-40s, when hormones fluctuate and symptoms begin. Menopause is one year with no period, on average around age 51. You do not need to know which stage you are in; your intake and clinician sort that out.

The 2002 study that raised concern used older formulations and studied women on average a decade past menopause. For many women, especially when started within about ten years of menopause, the benefits outweigh the risks, and in 2025 the FDA removed the boxed warning from menopausal hormone therapy. Your clinician reviews your personal and family history before prescribing.

Each suits different needs. The transdermal options, cream and patch, do not carry the clot risk associated with oral estrogen, and the pill is the simplest routine. Your clinician recommends the format that fits your health and preferences.

Hot flashes and sleep often improve within two to four weeks, with fuller relief by about three months. Individual results vary.

Bioidentical means the hormone is molecularly identical to the one your body makes. The estradiol and progesterone we use are bioidentical. Bioidentical alone does not mean safer; what matters is the right hormone, dose, and route for you, decided with your clinician.

If you have a uterus, progesterone protects the uterine lining when you take estrogen. Women without a uterus may take estrogen alone. Your clinician determines what you need.

Testosterone may be prescribed off-label for low sexual desire in menopausal women. There is no FDA-approved testosterone product for women, and major menopause and sexual-health societies recognize its role for low desire. Your clinician decides whether it is appropriate for you.

It may not be right with a history of breast, uterine, or ovarian cancer, blood clots, stroke, or certain liver or heart conditions. Screening for this is exactly what the medical review is for.

Newer guidance treats duration as a shared decision with your provider based on your symptoms and health, not an arbitrary cutoff age. You and your clinician revisit it over time.
