Levothyroxine, liothyronine, animal-derived thyroid and compounded T3/T4 are distinct preparations. A thyroid diagnosis and monitored replacement plan are essential; these medicines should not be presented as weight-loss or energy products for normal thyroid function. Levothyroxine replaces thyroid hormone and is widely used for hypothyroidism. Food, supplements and some medicines can affect absorption. Ask how lab follow-up and product changes will be handled. Excess replacement can cause palpitations and other symptoms and poses important health risks. 1

Compounded T3/T4

A compounded combination is not equivalent to an approved levothyroxine or liothyronine product by matching ingredient names. Its need, formulation consistency and follow-up require a specific review. A patient-specific 503A preparation is compounded by a state-licensed 503A pharmacy pursuant to a patient-specific prescription. Compounded drugs are not FDA-approved. 4

Taking and separating levothyroxine

Levothyroxine absorption can change with food, calcium, iron, antacids, bile-acid binders and several other medicines. Consistency matters: use the product-specific instructions and tell the clinician about brand, manufacturer or timing changes before interpreting a laboratory result. Biotin supplements can interfere with some thyroid tests, so disclose them before blood work. Do not compensate for a missed dose or a food interaction by changing the amount without advice. 1

T3 and desiccated thyroid require distinct decisions

Liothyronine acts more quickly and can produce higher peaks than levothyroxine. Desiccated thyroid contains both T4 and T3 in a fixed animal-derived ratio. Neither should be described as merely a natural or stronger version of T4. Palpitations, tremor, heat intolerance, anxiety, insomnia, chest pain or unexplained weight change may signal excess exposure or another problem. Combination therapy needs a specific rationale and monitoring plan rather than a symptom-only dose escalation. 2, 3

Compounded products and narrow therapeutic index

Compounded T3/T4 may be considered when an approved product cannot meet an identified medical need, but it is not FDA-approved. Potency variation is especially important for thyroid hormones because small exposure changes can affect symptoms and laboratory values. Ask for the exact ingredients, strengths, release design, pharmacy and reason for compounding. A slow-release claim needs formulation-specific evidence; it cannot be inferred from a capsule description. Pregnancy, heart disease, osteoporosis risk and interacting medicines can change the margin for error. 3, 4
