It is a group of changes that come with lower estrogen, including vaginal dryness, burning, pain with sex, and urinary urgency or frequent urinary infections. Unlike hot flashes, these symptoms often get worse over time without treatment. It is very common, and it is treatable.

Vaginal estradiol is a low-dose estrogen that works mostly right where it is applied. Vaginal DHEA is a hormone that the tissue converts into estrogen and related hormones locally. Your physician chooses between them based on your history.

Lubricants and moisturizers add moisture for a short time, but they do not change the tissue itself. Local hormone treatment helps the tissue become thicker and more elastic over time. Many women use both.

Many women notice some relief in a few weeks, with fuller change over a few months. It can take longer for pain with sex to improve. Individual results vary.

Some women notice local irritation, discharge, or spotting, especially early on. Because the dose is low and works locally, less reaches the rest of the body than with oral estrogen. Tell your care team about anything new, and report any unexpected vaginal bleeding.

Women with unexplained vaginal bleeding, certain hormone-sensitive cancers, or a history of blood clots may not be a fit. If you have had breast cancer, your physician may want input from your oncologist. Your physician reviews your full history first.
