An erection-related prescription is not the same as a lung-pressure treatment plan. Sildenafil lowers blood pressure and can interact with alpha blockers and other antihypertensives as well as nitrates and riociguat. The prescriber needs the exact names and timing of heart medicines, not simply a statement that blood pressure is controlled. Sildenafil supports an erection only with sexual stimulation; it does not create desire or treat every cause of erectile difficulty. A high-fat meal can delay the effect of some sildenafil products, while pulmonary-hypertension regimens follow different schedules. Agree on when to assess response before changing the plan. Persistent difficulty can be a clue to vascular disease, diabetes, medication effects, low testosterone or another condition requiring evaluation. 1, 2
