Flibanserin (Addyi)


Addyi is a prescription tablet containing flibanserin for acquired, generalized hypoactive sexual desire disorder in women younger than 65. The current label includes eligible postmenopausal women. It is not a general sexual-performance medicine, and other explanations for low desire must be considered before deciding whether the indication fits. 1, 2

What the diagnosis means
The labeled condition involves acquired, generalized low desire that causes distress or interpersonal difficulty and is not explained by another medical or psychiatric condition, relationship problems, or a medication or substance. An older premenopausal-only description is incomplete for the current label. 1
Safety and interactions
Addyi is contraindicated with liver impairment or medicines that moderately or strongly inhibit CYP3A4. Taking alcohol close to Addyi increases the risk of severe low blood pressure and fainting. Drowsiness and dizziness matter for daily activities. A medication review should include sedating drugs, supplements and medicines affecting flibanserin metabolism. 1, 2
What to clarify before treatment
Discuss what has changed, how much distress it causes, relevant medicines and whether pain, relationship factors or another condition needs attention. Ask what follow-up will assess benefit and tolerability, and what would prompt discontinuation. A prescription should follow that assessment rather than a symptom quiz alone.
Alcohol timing and interaction screening
The current Addyi label requires a specific alcohol-timing discussion rather than a general instruction to be careful. Drinking three or more standard alcoholic drinks in the evening means skipping the bedtime dose; after one or two drinks, at least two hours should pass before the dose, and no alcohol should be used until the next day. These are label rules, not a way to estimate personal tolerance. CYP3A4 inhibitors and liver impairment are contraindications because they can raise flibanserin exposure. 1, 2
Drowsiness and daily safety
Flibanserin can cause sleepiness, dizziness, low blood pressure and fainting. The label directs patients to avoid driving or other activities requiring alertness for at least six hours after a dose and until they know how they respond. Sedatives, sleep aids, antihistamines, opioids and other central nervous system depressants can add to impairment. If faintness occurs, lie down, seek help if symptoms do not resolve, and tell the prescriber about the event before continuing as usual. 1, 2
Reviewing whether it is helping
The approved diagnosis requires low desire that causes distress and is not better explained by a medical or psychiatric condition, relationship problem or medication effect. Follow-up should revisit those factors and the patient's own meaningful change, not a partner's expectation. The label recommends stopping after the specified trial period if there is no improvement. New depression, severe dizziness, fainting or medication changes should prompt earlier review. Compounded or differently routed flibanserin has not inherited Addyi's evidence. 1, 2
Stopping and restarting
Because interactions and alcohol use can change, an old prescription should not be restarted automatically after a long interruption. Review current liver history, medicines, supplements and alcohol pattern first. If a bedtime dose is missed, follow the label rather than taking it the next morning or doubling later. A partner's request or a temporary relationship stressor is not a substitute for confirming the diagnosis and the patient's own treatment goal. 1, 2
