Consent to Treatment

You consent to evaluation and treatment by a clinician of MDI Medical Group PC, the affiliated medical practice, for the condition you described at intake. Sipra LLC operates the platform and does not practice medicine. Section 4 of the Terms of Service at https://sipra.com/legal/terms explains that relationship.

Your clinician decides whether to prescribe, what to prescribe, and at what dose. Your clinician may prescribe nothing, prescribe something other than what you asked for, change your dose, or discontinue treatment at any time on clinical grounds. Paying for a plan does not entitle you to a prescription.

No clinician can promise a result. Outcomes differ from person to person and depend on the medication, the dose, and how closely the plan is followed. Nothing Sipra or your clinician tells you is a guarantee of weight loss, symptom relief, or any other outcome.

You agree to give complete and accurate health information, to disclose every medication, supplement, and allergy, to use the medication only as prescribed and only for yourself, to complete the lab work and check-ins your plan requires, and to report side effects promptly. Your clinician decides from what you tell them, so withholding information can make treatment unsafe.

You may refuse any treatment and stop at any time, without giving a reason, and doing so does not affect your right to care in future. Tell your clinician before you stop a medication so they can advise you how to do it safely.
