Recurring Charge Authorization
Effective: August 6, 2026
1What you are authorizing
You authorize Sipra LLC to charge the payment method on your account, on the schedule below, until you cancel. The authorization covers your membership, your medication plan, and any add-on you order. It applies to a credit or debit card, an HSA or FSA card, and to ACH and other electronic fund transfers (15 USC §1693e; 12 CFR §1005.10).
2When each charge happens
Charges follow Section 6 of the Terms of Service at https://sipra.com/legal/terms: your first month of membership is charged when you place the order, and medication and every later membership month are charged only after a physician approves. Your plan then renews at the end of each paid period, and each renewal is charged only after your physician approves the next refill.
3Amount
Each charge is the price for your plan shown above the pay button at checkout and repeated on your order confirmation. Prices change only as Section 6.11 of the Terms of Service allows. Charges appear as SIPRA*HEALTH.
4Revoking this authorization
Cancel at any time at https://sipra.com/account or by emailing [email protected]. Cancelling revokes this authorization for future charges from the next billing cycle and does not reverse a charge already made. To stop the next refill, cancel by the deadline in Section 6.5 of the Terms of Service. What is refunded, and what is not, is Section 7 of the Terms of Service and the Refund Policy at https://sipra.com/refunds.
5Errors and disputes
If a charge looks wrong, email [email protected] so we can resolve it. This authorization waives none of your rights; see Sections 6.10 and 6.13 of the Terms of Service.
6Record of this authorization
Checking the authorization box is your signature. We record the version of this document you saw, and the date, time, IP address, and browser. View your consents at https://sipra.com/account or request a copy at [email protected].
