Informed Consent for Genetic Testing

The ApoE genotype test reports which variants of the ApoE gene you carry (combinations of e2, e3, and e4). Your ApoE variants influence how your body handles cholesterol and how many physicians approach your long-term cardiovascular and brain-health risk. Because your genotype does not change, this is a one-time test that is never repeated.

This test is ordered by a licensed physician and performed by an accredited laboratory. It is intended to inform your care, not to diagnose a disease on its own.

Certain ApoE combinations are associated with a higher long-term risk of cardiovascular disease and of Alzheimer's disease. This is risk, not certainty: carrying a higher-risk variant does not mean you will develop a condition, and carrying none does not guarantee you will not. Your results should be interpreted by your physician in the context of your full health picture.

Genetic results can be emotionally significant. You will have the opportunity to review your results with your care team, and support is available.

Your genetic information is protected health information. Sipra will not sell it and will not share it with insurers or employers. In the United States, the Genetic Information Nondiscrimination Act (GINA) prohibits health insurers and employers from using genetic information to discriminate. GINA does not extend to life, disability, or long-term-care insurance, and some state laws provide additional protections.

Your results are stored in your medical record and handled in accordance with our Privacy Policy and HIPAA.

This test is voluntary. You may decline it at any time before your sample is processed without affecting any of your other care. If you have questions before deciding, you can raise them with your care team.

I voluntarily consent to genetic testing of my blood sample for the ApoE genotype. I have read this consent, I have had the opportunity to ask questions, and I understand that this test reports genetic information that does not change over my lifetime. I understand the results describe risk, not certainty, and that I may decline this test at any time before my sample is processed without affecting my other care. I authorize release of my results to Sipra and its ordering physician for my care.
