Use the first day of a supply and the pharmacy-confirmed days supplied. If directions changed, use is as needed, or the supply is a multi-dose vial, ask the pharmacy for dates instead of estimating. Entries are assembled into a downloadable worksheet. Marking a check complete records your own action; it does not certify a medicine, pharmacy or document.
Dates are calendar arithmetic, not a guarantee of medication availability or a determination of sterility, beyond-use date, coverage or refill eligibility. Ask your pharmacy about all applicable limits.

The worksheet can count calendar days only after you enter a reliable start date and days supplied. Changed directions, as-needed use and multi-dose containers may require the pharmacy to supply the dates.
